AMIDOSE CLINIC ELIGIBILITY AND ACCOUNT APPROVAL

1. BUSINESS APPLICATIONS
The portal is for approved clinic businesses, not public consumer purchasing or patient care. Applications are considered individually. AMIDOSE's Owner decides which business applications to approve; approval is not automatic and no fixed approval timeline is promised.
The applicant must be an owner, officer or expressly authorized business administrator able to act for the organization. Portal access approval is not certification of professional credentials or authorization under every applicable product or jurisdiction rule.
2. INITIAL INFORMATION AND CLARIFICATION
The initial application requests organization name, business email, business phone and the primary contact's first and last name. Do not submit patient information or PHI.
AMIDOSE may seek business-only clarification by email. Routine extra documents, license/tax identifiers or a public upload form are not part of launch intake. This notice establishes no additional document requirement.
3. DECISION COMMUNICATIONS
Clarification and approval/decline decisions are communicated manually through operational business email. Account activation instructions are provided through the enabled invitation process when available. A portal status change alone does not confirm an outgoing message or delivery.
4. CONTACT CHANGES AND CONTINUING REVIEW
Notify office@amidose.com of material contact or authority changes. Contact replacement requires manual review and confirmation of authority through an established business contact before an appropriate account change. There is no automatic account transfer or additional clinic-user account from this process.
Re-review is limited to material ownership/account changes or a credible discrepancy. There is no scheduled renewal or formal appeal process under this notice. The Terms describe manual access suspension and restoration.
